The National Transportation Safety Board concluded in a report Wednesday that faulty engineering caused an experimental submarine to explode, killing five people en route to the Titanic wreck.
NTSB The statement was made in its final report on the hull failure and explosion of the Titan submersible in June 2023. Everyone aboard the submersible died instantly in the North Atlantic when a catastrophic explosion occurred as the Titan fell into the wreckage.
The NTSB report stated that Titan’s faulty engineering resulted in “a carbon fiber composite pressure vessel that had numerous anomalies and failed to meet required strength and durability requirements.” It also said that Titan’s owner, Oceangate, failed to adequately test the Titan and was unaware of its actual durability.
The report also said that if Oceangate had followed standard guidance for emergency response, the Titan would have been found sooner, and this “would have saved time and resources, even though rescue was not possible in this case.”
The NTSB report matches a Coast Guard report released in August that called the Titan explosion preventable. The Coast Guard determined that the security procedures of Oceangate, a private company based in Washington state, were “seriously flawed” and found “clear discrepancies” between security protocols and actual practices.
OceanGate suspended operations and closed in July 2023. Company representatives did not respond to requests for comment Wednesday. Following the release of the Coast Guard report in August, a company spokesperson expressed condolences to the families of those killed.

The NTSB report recommends the Coast Guard commission a panel of experts to study submarines and other pressurized vehicles for human occupancy. It also recommends that the Coast Guard implement rules for vehicles informed by that study.
It also called on the Coast Guard to “disseminate the study’s findings to industry”, which has increased in recent years due to an increase in privately funded exploration.
The ship had been traveling to the Titanic site since 2021. Its final dive came on the morning of June 18, 2023. About two hours later the submersible lost contact with its support ship and a delay was reported that afternoon. Ships, planes and equipment were dispatched to the scene, about 435 miles (700 kilometers) south of St. John’s, Newfoundland.
A several-day search for survivors in Canada made international headlines. It soon became clear that no one would survive, and the Coast Guard and other authorities began a lengthy investigation into what happened.
