“The house failed Melissa in many ways,” he said.

“The resident who went to strangle her should not have been placed in the same facility as Melissa on the basis of their known risks.

“The decision was incorrectly made to keep in the same feature with an ineffective care plan and risk evaluation, with employees who were not trained at their risk levels, and the managers who failed to work on exposing concerns by employees and Melissa.”

The support workers at home told interrogation that they did not know about their history of hunter sexual behavior.

In a letter written before her death, Miss Maithison blamed social services for “destroying” her life.

The letter was read in court as part of the written evidence of Miss Maithison, who died of cancer a year after his daughter’s death.

“We blame those who kept her there with Melissa.”

Speaking after questioning, Mr. Mathison said: “My daughter should still be here and I miss her a lot.

“They reflect gravity of the failures of Alexandra home management, which were eventually responsible for the welfare of their inhabitants.

“In short, he welcomed a known sexual hunter at his care home with no risk management plan, at the same time a young woman accepted a young woman who matched her known victim profile.

“Even with these failures, the conclusion of the coroner correctly accepts the role played in Melissa’s death, which has occurred through his handle of Jason Carnoy’s infection in Alexandra Homes.

“It is strict sad that a young woman was deprived of her life as a result of these frightening, multi-agency failures.”
