Keanu Reeves has revealed that he almost went by the stage name Chuck Spadina, and even used Casey Reeves for a while before sticking to his real name.

Reeves said, “I got paid when I was 16, so I was a professional actor by the time I was 16, 17. I was in Toronto, Canada and then I got a manager who lived in Los Angeles.”
“At 20, I drove to Los Angeles in my car. Got out of my car and my manager said, ‘We want to change your name.’ so it’s welcome to hollywood [moment]I remember walking down the beach and I just said, ‘My name? What happens if I change my name? What?'”
Reeves lists the stage names he came up with, and none of them took off.
“My middle name is Charles, so I said, ‘…Chuck?’ And I grew up on a street called Spadina, [therefore] Chuck Spadina,” Reeves said. “And then I was some Templeton. So then I became Casey Reeves. I was credited as Casey Reeves.
According to his IMDb page, Reeves is actually credited as Casey Reeves in the 1986 anthology series magical world of disney,

“And then I couldn’t do it. So then I’d be at the audition and they’d go, ‘Casey Reeves.’ And I won’t even answer. After six months, I was like, ‘I’m not doing this.’ That’s a Hollywood moment.”
